
- Was your last eye exam more than one year ago (>40); two years ago (<40)?
- Do you wear glasses to see far away or close up? Can you see clearly (compare your vision with a known "eagle eye friend.")?
- Do you see poorer at night far away or close up?
- Are you taking any medicine? Is there a history of diabetes or eye disease in your family? Do you have a history of diabetes, high blood pressure, or eye disease?
- Do your eyes get fatigued or do you see double after prolonged computer or near work?
- Have you ever had surgery on your eyes? Or do you see double when you look in certain directions?
- Do you have trouble with judging distances? Far or near?
- Is one of your pupils larger than the other (look in a mirror or have a friend help you to make this judgement)?
- Cover your right eye lightly with your right hand and look at a small object more than twenty feet away - then move your right hand over to cover your left eye (uncovering your right eye to look at the same distant object). Did the object appear to move? Which way (same direction as your hand right-left or in the opposite direction)? How far did it appear to move (inches? feet?). Now repeat this test looking at an object at near (about 16 inches away).
- Do you have normal color vision?
